Aging well in today's complex healthcare system requires a coordinated program that links the resources available to seniors and their caregivers.

Inpatient Care

The hospital can be a challenging place for seniors if caregivers are not attuned to their special needs. BID Milton has committed to improving the hospital experience for elders by making geriatric education for staff a priority. BID Milton has partnered with Beth Israel Deaconess Medical Center’s Hospitalist program since 2007. Full-time, professional, trained hospitalists care for inpatients, ensuring that seniors get the specialized attention that they require. Geriatric consultation by two Harvard-trained geriatricians assists Milton’s hospitalists and primary care physicians with the management of complex age-related diseases. Social work and case management services ensure a smooth discharge process and enable provider communication at every level.

Outpatient Geriatric Consultation

Senior citizens are at high risk for geriatric syndromes including dementia, delirium, falls, functional decline, incontinence, osteoporosis, overmedication, and pain. Outpatient geriatric consultation by Harvard-trained geriatrician, Min Song, MD, assists BID Milton’s primary care physicians with the management of these syndromes, and ensures that our physicians deliver the most up-to-date elder care. This allows south shore seniors to benefit from the resources of the Harvard Division on Aging, while still maintaining a relationship with a primary care physician. Geriatric consultation can ensure healthy aging even when multiple complex medical problems are present.
