If you are on-site and seeking medical records from BID Milton, please proceed to the Highland Street Information Desk on the first floor and complete an Authorization to Release Protected Health Information Form. This will be processed and records sent to you via U.S. mail or email.

You may find the information you are looking for on our Patient Portal, MySite. Information related to the MySite Patient Portal can be found here. Once enrolled in MySite, you will find test results and other information.
If you require records for an upcoming appointment, please have your provider (doctor, NP, specialist etc.) fax a request to the Medical Records Department and we will send the records in a timely manner to the provider’s office.
If you are seeking imaging for services provided in the Radiology Department, please visit the Radiology Department on the Ground Floor of the hospital. They can provide images and a report of the study on a CD.
If you are seeking medical records from a Primary Care Physician or other physician office in the Lintz Medical Office Building (100 Highland Street, Milton), please contact their office. BID-Milton does not have access to these records.
