How to access your hospital records 

Your Medical Records 

You can access your medical records online any time through your patient portal. Your patient portal account includes test results and other details.

If you need copies of your medical records for an appointment, please have someone at your doctor's office fax a request to our Medical Records department at 617-313-1355. We will send your records in a timely manner to the doctor requesting the records.

If you need a copy of your medical records sent to another medical facility or for personal reasons, you must download and complete the Protected Health Information Release form provided by Beth Israel Deaconess Hospital–Milton.

You can send your completed medical records request form to us in several ways:

  • In person: At the hospital, turn in your completed form at the Highland Street Information Desk on the first floor 
  • By email: healthinfo@bidmilton.org

Requesting Imaging, Primary Care and Specialist Records

If you need copies of imaging tests, please visit our Radiology department on the hospital's ground floor. The radiology team can provide images and a report of your study.

If you need medical records from your primary care doctor or a specialist located in the Lintz Medical Office Building, please contact the doctor's office. BID Milton does not have access to those records.

Who Can Request Medical Records? 

You can request your own hospital medical records and those of a child in your care who is under 18 years old. 

Others who can access medical records include: 

  • Caregivers with power of attorney 
  • Estate executors of deceased patients (executor paperwork must accompany the record request form)
